Acknowledgement of Receipt and Compliance
I, the student, have read and reviewed all of the information provided in the Ereckson Middle School Rocket Club Rules and Guidelines packet. I agree to follow all rules and adhere to all guidelines that are specifically stated within the Ereckson Middle School Rocket Club Rules and Guidelines packet. Furthermore, I understand that if I fail to follow all rules and guidelines, my club membership may be suspended or terminated.
Parent/Guardian Permission to Participate
I, the parent/guardian provide my permission for my child to participate in the 2012-2013 Ereckson Middle School Rocket Club. I understand that my child will be required to attend all meetings and that I will provide transportation home after each meeting. I also agree to pick my child up after each meeting in a timely manner. I understand that the $20.00 registration fee is non-refundable.
