A Seizure Action Plan must be kept on file in the clinic.  A physician and parent signature is required.

An Asthma Action Plan needs to be on file in the nurse's clinic if your student requires an inhaler or nebulizer treatment during school hours.

An allergy action plan must be kept in the nurse's clinc for all students who must have Epi-Pen due to their allergy.
