Professional Reference Forms
– Print the forms

materials. It is your responsibility to distribute
the professional reference forms. The person must
return the form to you, the applicant, in a sealed
envelope with his/her signature over the seal.

A photocopy of a valid teacher certification or letter
explaining the status of teacher or professional
certification or license.

A photocopy of your college transcripts indicating degree(s) conferred.

SUBSTITUTES
After submitting
your application, transcripts, references and Substitute
Application Questionnaire, you will
receive a letter inviting you to a substitute
orientation when a date and time are set.
VOLUNTEERS
Please allow 2 WEEKS
for the application to be processed. After the HR
Department completes your background check, you will be
notified by email or by mail. Volunteers are required
to complete a new application each school year. You
will be able to use the same USER ID and PASSWORD each
year that you apply.
Applications remain
active for ONLY ONE YEAR, then inactive for one
year from the date of the application unless you update
your application. You will use your same User ID
and Password. If you do not update your application
within two years, your application and credentials will
be destroyed.
As a professional
