    7th Grade Immunization Records
     
    You may SCAN and EMAIL me your student's shot record over the summer!
    Do not fax it as I may not get it. 
    OR You may bring it to Ford Middle School Mustang Roundup.
    All 7th grade students are required to have the following vaccines BEFORE the start of school in August:
    • Tetanus, Diptheria and Pertussis booster (Tdap)
      Meningococcal (MCV4)
       
    •  ****DO NOT WAIT UNTIL THE WEEK BEFORE SCHOOL AS PHYSICIANS and
    • CLINICS RUN OUT OF VACCINES

    Family Access enables you to keep informed of your student's academic assignments and grades, as well as, health information.  Take a moment at the beginning of this school year to verify your student's health information in Family Access.  Should the information need to be updated, please notify me.  It is very important the school nurse has current information regarding student's health every year your child is in school. 

    Need a copy of your student's immunization record?  Access and print a copy directly from Family Access.
