MEDICATION: The Medication Permission form should be on file, filled out and signed by a parent/guardian, for any prescription or over-the-counter medications that may be administered to a student, in the clinic, during normal school hours.
ASTHMA: HB 1688, which passed during the 2001 Texas Legislative Session allows students to self-administer asthma medicaiton whle at school or school functions with permission from the parent/guardian and the prescribing physician. This is the district form, the physician may have a preferred form which would be acceptable. Any form used must be signed by the parent/guardian and the prescribing physician.
ALLERGY: An Allergy-Anaphylaxis Action Plan should be on file, filled out and signed by a parent/guardian, and the prescribing physician for any student who has a severe allergy, and may need an injection of epinephrine for an anaphylactic reaction, during normal school hours.
SEIZURE: A Seizure Action Plan should be on file, filled out and signed by a parent/guardian, and the prescribing physician for any student who is being treated for a seizure disorder, and may need seizure medication should a siezure occur during school hours.
